CDC guidelines for GABHS

Centor score (0-4) based on four clinical criteria:

• History of fever — 1 point
• Presence of tonsillar exudates — 1 point
• Presence of tender anterior cervical lymphadenopathy — 1 point
• Absence of cough — 1 point—

If 4 points, empiric treatment recommended—
If 3 points, either of two options recommended: ? Empiric treatment ? Rapid antigen testing; treatment only if positive—
If 2 points, either of two options recommended: ? Rapid antigen testing; treatment only if positive ? No testing or antibiotic treatment—
If 0-1 point, neither testing nor antibiotics recommended

• Modified approach adds 1 point for children <15>45 years old.
